If you are traveling with NLWI to Kenya, you and your team members each need to complete and submit this Medical Waiver Form. Please note that this is very important and that it has to be signed before a notary. Your local bank will have one. One completed, please send the form(s) to Paige at pcasey@nlwinternational.com.
